New patients, and previous patients who haven't been seen in the office during the current calendar year, must complete paperwork that includes demographic and insurance information, as well as a brief history of current symptoms, past medical and surgical history, medication and allergies, and social history.

You will need these forms, your driver's license or a picture ID, and your insurance card, even if your injury or condition is a workers' compensation, automobile accident, or personal injury claim.
